Registered Nurse - Oak Knoll Healthcare Center Per-Diem all shifts Per Diem 1: $41.00/hour Commitment includes 24 hours per week, weekend, 1 summer holiday and 1 winter holiday Per Diem 2: $38.00/hour Commitment includes 1 weekend a month, 1 summer holiday and 1 winter holiday Differentials offered for evening, night and weekend shifts:
Essential Duties: To provide exceptional care to all residents including but not limited to: - Obtain report from nurse being relieved and records sufficient information to implement appropriate follow up action as necessary - Provides report to nurse coming on duty, including sufficient information for follow-up action if necessary - Conducts resident rounds, reporting problems to nursing supervisor and initiating corrective action - Administers and documents medication, enteral nutrition and treatment per physician order and accurately records all care provided and observations daily - Performs comprehensive assessments of each resident on unit - Reviews and approves admission and re-admission nursing assessments as needed - Develops care plan based on each resident's assessment - Obtains physician orders and any revisions for all treatment and medications needed - Assesses on a weekly basis the skin condition, stage, size, site, depth, color, drainage and odor of condition - Renders immediate assistance to any resident in need - Supervise and evaluate all direct resident care and initiates corrective action as necessary - Provides clinical supervision of nursing assistants providing direct patient care - Assumes responsibility for compliance with federal, state and local regulations within the assigned unit - Counsels nursing assistants and communicates clearly with Clinical Nurse Managers and/or Director of Nursing if issues arise - Collaborates with interdisciplinary team to assure that high quality resident care is maintained - Prepares nurse assistant assignments and explains all pertinent resident care - Supervises distribution of nourishments - Supervises the removal of all postural supports - Orders, receives and documents medication according to facility procedure - Supervises response to residents calls for assistance and responds to call lights as necessary - Insures that all physician renewal orders and physician documentation is timely and appropriate
Prerequisites: - Graduate of an accredited Licensed Practical Nurse program or school - Current and active LPN license in the state of Massachusetts - Current certification in Basic Cardiac Life Support in accordance with AHS or ARC standards - Knowledge of Geriatric Nursing required
