Whittier Rehabilitation Hospital in Bradford, MA is looking for an experienced, highly organized Case Manager/Discharge Planner who thrives on being the calm, clear voice that guides patients and families through one of the most important transitions in their care journey. We’re hiring for a Full-Time (40 hours/week, M–F with rotating Saturdays) position. Part-Time (20 hrs/week) and Per Diem opportunities are also available. You must hold an active RN license in the state of Massachusetts.

What You’ll Do:
 
  • Oversee and coordinate the patient’s journey from admission to discharge, ensuring smooth, safe transitions every step of the way
  • Collaborate with the full interdisciplinary care team to set goals, remove discharge barriers, and maximize the effectiveness of rehabilitation
  • Communicate clearly with patients and families about their plan of care, available services, and what to expect post-discharge
  • Navigate CMS guidelines and managed care requirements while always advocating for what’s best for the patient
  • Provide customized resource options for the next level of care (e.g., skilled nursing facilities, home health agencies, etc.)
  • Maintain consistent contact with families, giving updates on care goals, treatment progress, and discharge planning
  • Anticipate disposition changes and stay ahead of potential challenges to ensure patients leave with the right support in place

What We’re Looking For:
 
  • Registered Nurse licensed in Massachusetts
  • 2–5 years of experience in acute care case management
  • Strong written and verbal communication skills
  • A sharp, organized mind with the ability to assess, plan, and evaluate complex care plans
  • Working knowledge of CMS regulations, managed care, and criteria for inpatient rehab or LTAC admission
  • A steady, professional presence in fast-paced situations
